没想到!出生时健康的宝宝,做了这个检查,确诊了遗传代谢病

高甲硫氨酸血症又称高蛋氨酸血症,是一种先天性氨基酸代谢异常疾病,遗传方式为常染色体显性遗传或隐性遗传。

菲菲出生时,并没有什么异常,菲菲父母带着她做了足跟采集血进行新生儿遗传代谢病筛查,令人意外的是,筛查结果提示串联质谱初次筛查发现甲硫氨酸(Met)为103 µmol/L(参考值:8.0~55.0 µmol/L),Met/Phe比值为2.27(参考值:0.16~1)。医生为了进一步确认,在菲菲出生12天后,召回重新采集血样进行复查,结果显示Met 152 µmol/L,Met/Phe比值4.48,疑似“高甲硫氨酸血症”。医生建议基因检测,结果提示菲菲MAT1A基因存在1个错义突变,被诊断为高甲硫氨酸血症。

菲菲的爸爸不能接受这个突如其来的结果,在走廊落下了眼泪。他怎么也想不明白,出生时还正常的孩子,怎么就得了罕见病?

MAT缺乏

医生介绍,高甲硫氨酸血症(isolated hypermethioninemia,MET),又称高蛋氨酸血症,是一种先天性氨基酸代谢异常疾病,遗传方式为常染色体显性遗传或隐性遗传。

甲硫氨酸代谢过程中任何一种酶相关基因突变,导致酶活性降低,影响甲基化反应,血中甲硫氨酸水平增高,而同型半胱氨酸降低。MET主要因基因突变造成将蛋氨酸转化成S-腺苷甲硫氨酸所需的甲硫氨酸S-腺苷转移酶(MAT)功能缺乏,而导致血液中的蛋氨酸堆积并升高,又称MAT缺乏症。肝脏疾病、酪氨酸血症Ⅰ型、经典型同型半胱氨酸血症、过多摄入甲硫氨酸等也可以引起继发性高甲硫氨酸血症。

MET发病率很低,国内外报道极少,台湾报道高蛋氨酸血症的发病率在1/106349。

临床表现

大多数患者无明显症状或仅有生化异常,亦有部分患儿表现为智力和运动发育迟缓、生长滞后、肌张力减低、新生儿胆汁淤积、牙齿和头发异常、心肌病等。少数患儿出现神经系统异常表现,如生长发育延迟、甘蓝样气味、呼吸有恶臭味。

辅助检查

1.血串联质谱中蛋氨酸指标明显增高,血浆同型半胱氨酸降低或正常,尿中也不会出现高胱氨酸,注意与同型半胱氨酸血症相区别。尿中甲硫氨酸浓度、4-甲硫基-2-氧代丁酸增高。

2.头颅MRI检查可发现大脑脱髓鞘病变。

3.基因检测是诊断的金标准。检测MAT1A、GNMT、AHCY基因。肝脏活检MATⅠ/Ⅲ活性测定也可确诊。

治疗

目前此病尚无特效治疗方法,主要通过服用辅助代谢的药物和特殊医学配方奶粉、严格控制饮食来维持身体发育。如补充S-腺苷甲硫氨酸,定期检测生长发育、智力发展,血中甲硫氨酸浓度,以便及时了解饮食控制效果。也有建议给予AdoMet,有报道患者使用后大脑髓鞘重新形成。摄入肌酸和胆碱可为辅助治疗。

高甲硫氨酸血症的遗传方式为常染色体显性遗传或隐性遗传,是可防可治的遗传代谢病,早期诊断和早期干预对于改善预后至关重要。
