非酒精性脂肪性肝病(NAFLD)分原发性和继发性两大类,前者与胰岛素抵抗和遗传易感性有关,而后者则由某些特殊原因所致,比如体重。
非酒精性脂肪性肝炎 (NASH) 是一种慢性进行性的非酒精性脂肪肝病 (NAFLD)。NASH 估计影响全球 1%–6% 的人口。到 2030 年,欧洲、美国和中国的 NASH 病例预计将增加 40% 以上。NASH 中的肝细胞损伤和炎症与纤维化有关,这是导致发病率和死亡率恶化的最强独立危险因素。一项系统评价和荟萃分析显示,在中位 6 个月时间内,体重减轻与血液标志物和脂肪变性改善之间存在剂量反应关系。但体重变化对疾病活动性的影响仍不清楚。因此,本项研究旨在探究体重变化(增加/减少)与 NASH 的生化和组织学特征变化之间的关联。
这是对 NASH 成人患者的研究队列,所有患者在基线和随访至1.5年或2年时进行了肝活检。多变量回归模型检查了体重变化如何与 (a) 血肝标志物、(b) NASH 消退而没有纤维化恶化、(c) 纤维化改善而没有 NASH 恶化和 (d) 个体组织学特征的变化进行了相关性分析。
研究结果发现421 名参与者的平均BMI为 34.3 kg/m 2,他们的平均体重变化为增加0.5 kg 。体重变化与肝酶变化和 Fibrosis-4 评分独立正相关(P < .001)。每公斤体重减轻与 实现 NASH降低7%相关(95% CI, 3%-10%; P < .001),并且可以降低 5%的纤维化改善且 NASH 没有恶化的几率 (95% CI, 1%-8 %;P = .01) 。体重增加与疾病活动的恶化有关。每减轻一公斤体重,纤维化改善的几率为 5%(95% CI,2%-8%;P = .001)。
